If you kow the name and address of the property you are staying in then I would put US stay and hotel details.

Well, assuming you're staying at that hotel for the first night of your trip (and by the sound of it for a while after, also), you should put the address of your hotel.
From the
FAQ section of va.com:
The additional information includes your country of residence and the address of your first night's accommodation in the US if you are not a US Citizen or resident.
More info
is also available here.

Hi
Take along a hard copy showing your booking confirmation as we had to show proof of our reservation at MCO immigration.

We put "staying in the US" for our trip. We stayed 12 nights in Orlando and a night in NY. Currently in the JFK clubhouse waiting for dinner and VS010. Going to miss this holiday (sniff).

If you are on a fly drive and do not know where you will end up then put the name of the hotel at the airport!!! How are they gonna know?

They're not - but you'd better hope they don't take a random sampling for checking.
You'll probably be in shackles the next time you visit [:#]

If you don't know where you're staying instead of making it up it is perfectly acceptable to put accomodation on arrival and give a general itinery such as "Staying in Kissimee area for two weeks".
Same if you back packing - a general itinery is fine.
