Questions re: LHR Revivals Lounge

When my wife and I land at LHR on our first trip on Virgin, we plan to take full advantage of the revivals lounge. I have a few questions, however, about the revivals lounge and would appreciate any information anyone has to offer.
BOOKING TREATMENTS
When, where and how do you book treatments like neck massages? Is there a best time to book the treatments? Are any of the treatments complimentary or is there a charge?
FOOD
We will be landing in the morning. Do you receive a better breakfast in the revivals lounge or on the plane?
LUGGAGE
Do we pick up our luggage first and go through immigration before we enter the revivals lounge? How far will we have to carry our luggage to the revivals lounge?
GROUND TRANSPORTATION
Since we are using frequent flyer miles for our UC tickets, we will not receive the complimentary Virgin limousine service. [:(] Since we do not know how long we want to stay in the revivals lounge, we are leery of booking a private car in advance as any such service will want to know exactly what time to pick us up. Are the lines for black cabs at Heathrow usually bearable? If so, how far are the taxi stands from the revivals lounge? Are there any private car companies that can be hired on a moment's notice (preferably via a telephone call from the revivals lounge) without entailing a wait?
