Record Locator, Reservation Code, Booking Reference and Confirmation Number are all used to access the PNR (which is not the code itself, but the actual information about the passenger and itinerary).

I can't speak for the VS system, but the DL system generates the record locator at the time that the PNR is built, which occurs way before booking. You can actually create a PNR with nothing more than a passenger name and phone number, and depending on how the system works either updates as each flight is added, or after all of the flights have been added. Then the PNR is fared, form of payment entered, and transaction confirmed.

We used to see loads of record locators that don't really go anywhere. As an example, if a Northwest flight was delayed and they started backing passengers up on one of our flights, the NW PNR would show up on our passenger list, but sometimes was nothing more than a name and phone number if they hadn't actually booked the passenger onto a flight yet.

You'll notice if you book a CO/VS itn with both airlines you get a CO record locator and a VS record locator as the two systems don't talk together very nicely, so the PNR's aren't readily accessible and thus each airline's portion shows up on their own system.
