Isn't this the same type of error that nearly caused a catastrophic crash of an EK A345 at MEL last year? It seems that these mistakes are not uncommon as the report points out - I'd be happier if I heard some serious CAA or FAA wrist slapping of airlines and a serious focus on preventing this from happening again!
