Approx $50-$55, not including tip.
It's a flat-fare of $45 from JFK to Manhattan, but this doesn't include tolls, which are usually $5 or so, depending on your route. Then I think they change $1 per piece of luggage or something.
I usually reckon on $60 all in.
Yup, $60 is a good estimate for a cab from JFK to Manhttan all in.
You could also take the Air Train as a cheaper alternative.

The cheapest option is the A line subway for $ 2.25 - it takes about an hour to get to 42nd Street.
(A day pass costs $8.25 and a 7 day one $ 27.00)
The shared shuttle buses are about $20 per person

But to get to Howards Beach subway station you need to take the Airtrain which costs $5.00 so $7.25 in total. Still a million times cheaper than a cab with multiple tolls and "tip".

The last time I did the A line route the Air Train was atill being constructed and I remember getting one of the free airport shuttle buses to Howard Beach Station.
Though I did this as a 'try it once to see what it was like'.
Next trip I graduated to the Shuttle Bus and then more recently to taxi (just like going Econ to PE to UC - once you have upgraded there is no going back !)

I've used all the methods to get in and out of JFK:
Bus
Taxi
Hire car
Airtrain and Subway
Airtrain and Long Island Railroad
If there's a group of you then a taxi does make sense however nothing will beat the price and the speed of the Airtrain for getting in/out Manhattan especially if you are by yourself. There comes a point when the taxi fare (even if it is a flat rate like from JFK) is too stupidly expensive and a complete rip off.

Ahem, limo, helicopter????
I can't for the life of me work out why anyone wants to take the connecting trains, with luggage, only to likely have to take a taxi when they reach Manhattan!
There's a plane at JFK, to fly you back from far away
all those dark and frantic transatlantic miles
