You just turn up. When you check in you will be given directions as to how to get to the CH and when you arrive you will be asked what flight you are on and asked for your boarding card which will then be checked against the list of people entitled to access.

If I have an UC ticket, even if it is purchased with partner miles ( BMI Destination Points), will I still be eligible to use the CH at LHR?

A UC ticket is a UC ticket regardless of how it is come by. The only time you do not get CH access is if you get an op-up (operational upgrade to the uninitiated) at check in
