New I-94W form

I picked up a stack of the new design forms on my last trip, just come to fill it in and haven't a clue what to put in box 12(City where visa was issued)and 13(date issued).Its a visa waiver so why would I have a visa.[?]

Its seemingly a printing error [ii] - just leave them blank.

This was pointed out & posted in May - after the party bus

Leave 12 and 13 blank as these were printed in error.

There is now new form in issue.

Just to add that the Forms have been once again reprinted[n]

Both the forms issued at the check in Desk and available at the Clubhouse had the lines removed.

Immigration accepted both
