I am planning on using my miles for a UC reward flight later this year, I almost have enough flights for me and the wife to fly both ways. But I need a top up of 15k Miles, can I book using her miles as the top up as she will be the other passenger or do I need to go through the pain of a transfer of Miles?
You can use miles from seperate accounts, but only in amounts that will cover one sector - e.g. if the return trip is 100K miles, you could do 50K from both accounts, but not 85K from one and 15K from another.
No you can't "pool" miles to make up a shortfall. Legs have to be "paid for" in their entireity from one account so you'll either need to buy , transfer or accrue the 15K shortfall.

Thanks. Would that mean effectively booking one return seat and two one way seats? As the increase taxes cost of two one-way seats negates the price of miles transfer.
She has enough for one way UC so If that is the case I will need to do some number crunching.

If she has enough miles for one outright leg that'll be fine. You can obtain 2 return tickets by "paying " for three legs from your account and one from hers

DL, that seems to have added costs when I tried online as one-way taxes are more than a return.
Unless I can do it over the phone.

yes best do it over the phone.
Your good lady will first have to give VS authority for you to use her miles then you can do the booking. This is quite common so won't flummox the agent too much !

Huzzah for International Jet-setting !
and allowing you to use her miles means you won't have to pay any transfer/gifting charges as well
