Has your e-ticket been reissued yet? If not and if it's been 72 hours since you made the call, call again to check it.

In my experience it happens when the flight is ticketed. In straightforward bookings (e.g. book a reward flight through the website) that's pretty much straight away. In the past when I've tried to upgrade or change an existing booking then it's been referred to a different department for ticketing, and both the extra cash & miles have taken a few days to be debited, and likewise I've had to wait that time for the revised e-ticket to be sent.
