I've also had the same e-mail today, not filled it in yet though.
I've not had an email but completed an on board questionnaire recently. Unfortunately, the questionnaire didn't ask some questions it should have and didn't give space for comments.

I've now had one of these emails. All multiple choice questions so they can pretty much control the info they get back, no space for actual valuable comments in case they get something they don't want to deal with. Very bad customer survey IMHO v(
